1. All Regular and Special Members of SLAGA are covered by the DAF.
2. The annual contribution amounting to Php240 shall be deducted from dividends due at the end of the fiscal year or paid over-the-counter at the SLAGA Office.
3. Claims for DAF by legitimate heirs or beneficiaries shall be supported by documentary requirements of SLAGA for claims of deceased members. (Check the Death Claim Requirements for the list)
4. DAF may be used to pay for any outstanding loans, past due accounts, and interests and surcharges due from deceased members unless shouldered by loan co-makers.
5. Members whose Capital Contribution have been transferred to Accounts Payable and who are no longer entitled to dividends cannot avail of the DAF.
6. New members shall pay the annual contribution in full regardless of the date of membership.

1. Notarized Affidavit of SLAGA Claim and Settlement (See sample format below)
2. Death Certificate (Original or Certified True Copy)
3. Valid IDs of the deceased SLAGA member and the legal heirs-claimants (Photocopy)
4. Birth Certificate of children-claimants (Original/Certified True Copy). In case of descendant-claimant, Birth Certificates of descendant and his parent (child of deceased) shall be submitted.
5. Marriage Certificate of widow/widower-claimant (Original/Certified True Copy)
6. Birth Certificate of the deceased, if claimant is parent (Original/Certified True Copy). In case of ascendant-claimant, the Birth Certificates of the deceased and that of the parent of such deceased shall be submitted.
