Membership
A. Under the provisions of Sections 1 and 2 of the SLAGA By-Laws the following may become members of the Association:
- A COA or SLAGA employee who applies for membership and continues to contribute to his capital to the extent authorized by the Board.
- A retired COA or SLAGA employee who, within the prescribed period, signifies his intention in writing to continue his membership.

- Duly accomplished Membership Form
- 1×1 ID picture
- Attested Appointment (photocopy or scanned)
- Latest pay slip (photocopy or scanned)
- COA ID (photocopy or scanned)
- Payment of the initial Capital Contributions and other fees (see payments tab)
Note: You may submit your application of membership through e-mail ([email protected]) or at the SLAGA Office.

Initial Capital Contribution: Php 3,000
Membership Fee: Php 100
Death Assistance Fund: Php 240
TOTAL PAYMENT UPON APPLICATION OF MEMBERSHIP: Php 3,340
